Order materials for your
Guest at Your Table service

UUSC will provide all the materials needed for your church's Guest at Your Table service and program free of charge. Print, complete and mail or fax the printable order form, complete and submit the online order form below, or call UUSC at 800-766-5236 to request your materials. If you prefer, you may download the materials in PDF format and print them yourself.

Please allow four weeks for delivery of your materials. Rushed delivery will incur an estimated charge of $25 to $50.
 
We are not able to fill any orders for materials to be received by Sunday, Nov. 18.

ONLINE ORDER FORM
Please complete the form and submit below, using the following guidelines to ensure that your materials arrive in plenty of time for your box distribution/Sunday service.

  1. Please check the appropriate box below to indicate whether you prefer UPS or U.S. Postal Service delivery. The cost to the Service Committee is generally higher when shipping via U.S. mail. For UPS delivery, please provide a street address at which someone will be present between 9:00 a.m. and 5:00 p.m., Monday through Friday.
  2. Please order materials at least 4 weeks before the Guest at Your Table box distribution date.
  3. Please note that a shipping charge of $25 to $50 is estimated for rush orders. If you have any questions regarding delivery, please call 800-766-5236.
  4. Provide a phone number in case of questions regarding your order.
  5. Please include your (UUA) church code on the order form. The code appears above your name on the mailing label on your coordinator's packet. If you don't know your church code, you can find it here.

For congregations of less than 200 members: limit 10 of each fact sheet
For congregations of 200 to 449 members: limit 20 of each fact sheet
For congregations of 450 and more members: limit 30 of each fact sheet
