Please complete the online order form below, using the following guidelines to ensure that your materials arrive in time for Justice Sunday, March 30, 2008 (or the date on which you choose to hold your Justice Sunday service).

Guidelines

  1. Please check the appropriate box below to indicate whether you prefer UPS or U.S. Postal Service delivery. The cost to the Service Committee is generally higher when shipping via U.S. mail. For UPS delivery, please provide a street address (no PO boxes) at which someone will be present between 9:00 AM and 5:00 PM Monday through Friday.
  2. Please submit this form at least 2 to 4 weeks before your service date, and indicate the date of your service. (Especially if your Justice Sunday date will not be March 30, 2008)
  3. A shipping charge of $25-$50 is estimated for rush orders. If your service is less than 2 weeks away, please contact Shari at orders@uusc.org or 617-868-6600 immediately to make arrangements for rush delivery. Do not use this online form for rush orders.
  4. Please provide a phone number and e-mail address at which you can be reached in case of questions regarding your order.
  5. After you have filled out this form, before clicking "Send," please print it for your records.
  6. Please provide your 4-digit UUA church code on the order form. If you don't know your church code, you can find it here.
