Make a memorial or commemorative donation

Use this form to make a donation in memory or in honor of a loved one.

A "memorial donation" is a gift given to UUSC in memory of a deceased friend or loved one. As the donor, you will not earn membership through this donation. You will receive an acknowledgement of your donation. If you choose to provide UUSC with the name and mailing information of a family member or other loved one of the deceased, UUSC will send them a card notifying them of your gift.

A "commemorative donation" is a gift given to UUSC in honor of a friend or loved one, often to commemorate an anniversary, birthday, graduation or holiday. As the donor, you can earn membership through a commemorative donation if it is of the correct membership amount ($25, or $10 for student, youth or seniors). The honoree will not receive membership. You will receive an acknowledgement of your donation. If you provide us with your honoree's name and mailing information, we will send them a card to notify them of your gift.
